Recreation Job Opportunities.

Default Notification Form

The responsibility of a team captain extends far beyond registering a team. Intramural captains are a vital part of the Intramural Sports Program and without their efforts to organize their teams, the program would not succeed.

Team captains have a unique opportunity to acquire leadership, communication, and decision-making skills! Please be sure to review the information below and share it with your teammates. Good luck, have fun, and always display good sportsmanship.

Notification Deadlines:

  • Games scheduled Monday – Friday = 1:00pm the day of the game
  • Games scheduled on Saturday- Sunday = notification must be received by 1:00pm the Friday prior to your game.

This form must be completed to notify the Competitive Sports office that your team is unable to play in an intramural game. Not notifying the Competitive Sports office by the appropriate time will result in a $10 Forfeit Buy-Back Fee for individual sports, or a $25 Forfeit Buy-Back Fee for team sports. If a forfeit occurs, this fee must be paid by the team captain to allow your team to rejoin the league. Forfeit Buy-Back Fees may be paid either online through Fusion, or in-person at the RAC. Your submission below will be time-stamped.

Only team members may submit forfeit notifications
